*For male insured persons, 25 years old, in good physical condition; premiums paid annually, career group 1,2; Plan 1 coverage (premium rates vary with increasing age)
**Health insurance rider with separate expenses – Health Care Plus – for inpatient fees, including room, meals, nursing care, surgery and medical other procedures, as well as emergency treatment, premiums are to be paid until the age of 79, but coverage to the age of 80
*** Health insurance rider with fixed expenses – Health Benefit Plus – premiums to be paid until the age of 64, but coverage to the age of 65
****Insurance rider – Multiple CI Plus – premiums to be paid until the age of 79 with coverage to age 80; compensation to be made upon diagnosis of any of the included critical illnesses
